Welcome, Welcome, Welcome!!!

We are very excited that each of you and Camp Horizons found one another and that we will be spending the summer of 2004 together!!!

Beth Mucci, the Assistant Director of Staff Services, will be sending you a packet of information about camp, your responsibilities & life in the United States! Please, please, if you have questions, email me and ask! We are here for you, we now consider you part of the Camp Horizons family!

There are 4 important documents we will need back from you that will be in the packet of information we are sending:

1. The LETTER OF AGREEMENT-We send you 2 of them, one is for you to keep for your own records, the other is for you to sign and send back to us.
2. The HEALTH HISTORY form-This is a form that you can fill out yourself and sign yourself and send it back. This and all personal information is treated as confidential and will be secured at camp and shared only with our medical personnel.
3. The PHYSICAL FORM-This must be completed and signed by a doctor. We will accept the medical physical form that your agency makes you & a doctor complete in place of ours as long as it is in English and signed by a doctor. We would still need you to complete our Health History form in addition to any physical form.
4. The PRECAMP STAFF QUESTIONNAIRE-This is to be completed by you. This is to give us an idea of your experiences and let's us know what type of training we should plan for the staff of 2004.

We also request a picture, we love receiving your pictures!!! We can take one of you when you get here to post on our staff wall, but we like to have as many ahead of time as possible.

I will try to update this section of our website as often as possible with information and hopefully some helpful hints about how to complete forms, travel to camp, as well as news about the upcoming summer.
